Fraser Health is looking for a Community Health Nurse - Registered Nurse to join our Home Health team. In this role, you will work independently and with a committed interprofessional team to manage client care, conduct thorough assessments, and promote effective health interventions for chronic conditions. Your contributions will support clients’ journeys toward better health within their communities.

Key Responsibilities:
• Initiate and nurture therapeutic relationships with clients
• Perform comprehensive assessments and care planning
• Provide direct care and coordinate additional health services




• Ensure effective transitions among healthcare providers
• Advocate for client preferences and informed decision-making

Requirements:
• Valid registration with BCCNM as a Registered Nurse
• One year of related clinical experience in community settings
• Current BC Driver's license and personal vehicle
• Solid assessment and therapeutic intervention skills
• Effective communication with diverse populations
