NHS Scotland is seeking an experienced Health and Social Care Worker to join the Community Rehabilitation Team in the Dunfermline locality. You will deliver rehabilitation programmes in service users' homes or in community settings, working with OT and Physiotherapy colleagues to prevent hospital admissions and support early discharge.
The role requires SVQ Level III in Health and Social Care (or equivalent). Experience as a support worker and with older people is desirable.
