Liquid Personnel is looking for experiencedOccupational Therapist to support its client's Adult Social Care service tea in Rutland, delivering high-quality interventions to enable individuals to remainindependent in their own homes.

This is a varied role, combiningreablement, discharge support, and community-based interventions.

What will your responsibilities be?

Completeholistic, strengths-based assessmentsand develop personalised reablement plans

Support individuals onDischarge to Assess pathways, enabling safe return home

Work with community patients toprevent hospital admissions

Assess and provideequipment and adaptations(including major adaptations)

Completerisk assessments, including complex moving & handling cases

Promote and support use ofassistive technology

Work collaboratively withhealth and social care professionals

Maintain accurate case records and monitor support plans

SuperviseOT Assistantsand provide guidance to colleagues

Qualifications & Experience:
To be successful in this role you must have,

Degree/Diploma in Occupational Therapy

HCPC Registration

Experience withinAdult Social Care or Health settings

Understanding of:

Care Act 2014

Safeguarding procedures

Strong assessment, risk management, and care planning skills

Key Skill:

Ability to manage and prioritise workload effectively

Strong communication and partnership working

Confident inmoving & handling assessments





IT literate (case management systems, email, data input)

Working Pattern:

Hybrid working:

2 days office-based

1 day working from home

Remaining time in the community
