This full time contract role is part of the HART Hub initiative. You will be responsible for daily program facilitation, providing brief case management, and offering crisis support. Flexibility in work hours, including weekends and evenings, is essential to meet client needs.

Key Responsibilities:
• Lead daily groups aimed at wellness and community engagement
• Engage with interdisciplinary teams for holistic care
• Deliver education and facilitate resource referrals
• Support clients in accessing essential appointments




• Provide overdose prevention education and response

Requirements:
• Post-Secondary education from a recognized institution
• 3-5 years of community health work experience
• Experience in facilitating groups and assessments
• Comprehensive knowledge of crisis intervention strategies
• Effective communication and teamwork capabilities
