Key Responsibilities:
• Manage cases and support patients with diabetes
• Assess needs and collaborate on care plan development




• Provide crisis intervention and community resource referrals

Requirements:
• Degree in Social Work or equivalent education
• Registration or eligibility for OCSWSSW
• Eligible for Canadian Diabetes Educator Certification
