You will join the Community Care, Mental Health & Addictions program, focusing on intensive case management for seniors facing mental health and addiction issues. Your responsibilities will include conducting psycho-social assessments, developing service plans, and providing necessary support to clients and caregivers. Key Responsibilities:
• Conduct psychosocial assessments and care planning
• Provide crisis intervention and risk assessment




• Offer ongoing case management and counseling solutions
• Lead outreach initiatives for community awareness
• Maintain accurate documentation for client services

Requirements:
• Master’s in Social Work (M.S.W.) or related field
• Registered and in valuable standing with OCSWSSW
• Over 3 years of experience in relevant fields
• Deep understanding of geriatric care
• Proficiency in trauma-informed care principles
