As a Case Manager, you will be responsible for assessing youth needs and developing tailored care plans. This role emphasizes trauma-informed care, crisis intervention, and collaboration with various internal and external services. You will also facilitate workshops and advocate to ensure youth access essential resources in social services and housing.
Key Responsibilities:
• Conduct assessments and develop individualized care plans
