Your role will include participating in goal planning and connecting individuals with necessary community resources for optimal support. Key Responsibilities:

- Facilitate participation in community-based activities
- Involve in individual support planning and resource identification
- Provide personal support aligned with individual goals
- Act as a role model in the community
- Comply with safety measures and maintain confidentiality Requirements:
- Degree or diploma in developmental services or similar
- One year experience in community support settings
- Valid First Aid, CPR, and NVCI certifications
- Strong interpersonal and computer skills
