Circle of Care, Sinai Health is seeking a Full time Community Social Worker on a 12-month contract to collaborate with hospital and community partners, providing linkages with rehab options, home and community support services, geriatric care navigation, and caregiver support.
Based at Head Office with regular community travel, this role reports to the Manager, Social Work, ADP, Hospice and Bereavement and offers a salary range of $65,000 to $76,500 per year, depending on experience.
