Career Opportunity
We are looking to hire two Service Coordination Professionals (BSW) full time, to join our Halton Support Services team! In this position, you will play an important role in ensuring the provision of appropriate social and community support services to maximize the potential for independence for people with developmental disabilities living in community settings or with their families.
RESPONSIBILITIES:
- Provide person-centred service coordination and case management through Adult and Children’s Programs for individuals with developmental and intellectual disabilities and/or complex special needs.
- Conduct biopsychosocial assessments to identify needs, risks, strengths, and required supports.
- Develop, implement, monitor, and evaluate individualized support plans in collaboration with individuals, families, caregivers, healthcare professionals, and community partners.
- Build supportive, respectful relationships that honour the values, preferences, and goals of individuals and their families.
- Identify and respond to crisis situations using appropriate prevention and intervention strategies.
- Connect individuals and families with appropriate funding, community resources, healthcare providers, and social services.
- Advocate for individuals’ rights, social inclusion, and access to appropriate services and supports.
- Support individuals and families with problem-solving, daily living skills, and navigating community resources.
- Facilitate communication, problem-solving, and resolution among service providers, families, and community organizations.
- Apply trauma-informed, anti-oppressive, and evidence-informed approaches in service delivery.
- Maintain accurate and comprehensive case documentation, correspondence, records, and service statistics in accordance with policies and procedures.
- Maintain knowledge of relevant legislation, policies, and the broader social service system affecting individuals and families.
- Build and maintain cooperative relationships with community agencies, professionals, and community leaders.
- Provide public education, workshops, and information sessions for families, caregivers, community agencies, and the broader Halton community.
- Conduct outreach to individuals who may have limited access to services and connect them with appropriate supports.
- Contribute to the development and maintenance of internal and external community resource systems.
- Participate in internal and external committees and contribute to the ongoing development of service delivery.
- Participate in regular clinical supervision, provide case updates, prepare materials, and follow through on action items.
- Perform other duties as assigned.
WHAT QUALIFICATIONS AND SKILLS DO YOU BRING?
- Bachelor’s Degree in Social Work (BSW) from an accredited institution.
- Registration and membership in good standing with the Ontario College of Social
- Workers and Social Service Workers (OCSWSSW) is required.
- Demonstrated experience in case management, assessment, and working with people of all ages with developmental disabilities in a community setting
- Minimum of 2 years of relevant experience required
- Knowledge of social services agencies in Halton Region and an understanding of the social services system and the systems
- Ability to work within a team, with other support providers, families, volunteers and community members and to maintain helping relationships.
- Excellent written and oral communication skills, crisis intervention and counseling skills.
- Self-initiative and ability to work independently.
- Excellent organizational skills and ability to balance administrative duties with direct service delivery.
- An ability to work flexible hours,
including evenings and weekends as needed.
- A valid Class “G” Drivers License and a willingness to drive.
- Providing proof of vaccination for COVID-19 and maintaining vaccination status as necessary from time to time during employment.
