Reports to and works under the direction and supervision of the Ontario Works Case Manager Team Lead.

Role Details

Hours of Work: 35hrs/week

Wage: Minimum $63,200.00/year A competitive compensation package will be offered commensurate with qualifications.

BASIC QUALIFICATIONS

University or College Diploma in Social Work/Human Services or related field with one (1) year of experience in Social Assistance or related Human Service setting; OR

Ontario Secondary School Diploma or Equivalent with at least three (3) years of experience in a Social Assistance or related Human Service setting

Knowledge of the Ontario Works Act, Regulations and Policy Directives

Valid driver's license and access to a reliable vehicle

Successful completion of a criminal background check

POSITION TITLE

Ontario Works Case Manager

REPORTING RELATIONSHIP

Reports to and works under the direction and supervision of the Ontario Works Case Manager Team Lead.

PURPOSE & SCOPE OF THE POSITION:

The Ontario Works Case Manager is responsible for providing comprehensive case management services to individuals and families receiving assistance under the Ontario Works program. This includes evaluating clients' needs, formulating personalized service plans, and connecting clients with necessary resources and supports to foster financial independence and self-sufficiency. The role also entails thorough documentation within the information system, encompassing assessments, monitoring, as well as detailed progress notes and the creation of client-specific and caseload reports.

The Ontario Works Case

Manager plays a crucial role in supporting individuals in need within the framework of the Ontario Works Act & Regulations.

KEY DUTIES & RESPONSIBILITIES:

Technical Functions

Detailed knowledge of Ontario Works Act, Regulations and Directives

Completes application forms for assistance

Conducts detailed assessments of employability needs and barriers for clients, spouses, and adult dependents

Prepares and makes referrals to community programs, external agencies, and clinical assessments based on needs assessment results

Develops phased participation plans with clients to address employment barriers, including internal and external services like Skills Acquisition and Training Fund (SATF) projects

Monitors continued eligibility for financial assistance and client participation

Schedule regular meetings with clients for supportive counseling, skills training, and progress reviews

Monitors client participation in referral programs and maintains regular contact with these programs

Schedule formal review meetings every three months to assess progress and update participation plans





Participates in client orientation and group work sessions as required

Administrative Functions

Prepares referral documentation and participation reporting requirements for clients and referral programs/agencies

Prepares monthly budget worksheets based on income statements

Maintains accurate and confidential client records in compliance with program policies and regulations

Ensure timely and accurate processing of financial assistance payments

Records the results of assessments conducted and plans developed on the basis of these assessments (goals, progress indicators, service assignments, referrals, placements)

Records all contacts with and about clients with notes arising from these contacts in the information system

Updates existing client information as appropriate (e.g. education history, financial resource history, external service providers, documents received, phone numbers, addresses)

Prepares or participates in the development of statistical/financial reports as directed

Determines, in consultation with the client and the Ontario Works Case Manager Team Lead, when a temporary and/or permanent deferral from participation is warranted in an ongoing case

Attends and participates in administrative reviews of client complaints and any subsequent provincial tribunal or court appeals as directed

Prepares and responds to electronic, faxed and written correspondence as appropriate and in a timely manner

Attends staff meetings, workshops and training as required

Communications Functions

Receive and respond to telephone inquiries from clients and other agencies

Advocate on behalf of clients to ensure they receive the necessary supports and services

Provides written notification to clients as required in relation to changes in participation support and lack of compliance

Reports any failure of a client (recipient, spouse or adult dependent) to comply with participation plan requirements to the Ontario Works Case Manager Team Lead

Facilitates workshops and information sessions as required

Other Functions

Performs other related duties as assigned

Ensures compliance with Ontario Works policies, procedures, and legislative requirements

Required to travel to meet clients and attend community meetings

WORKING CONDITIONS

The work environment is subject to high public profile, and interaction and engagement with the public who may be hostile and/or irate The work may be subject to quick deadlines, interruptions, and unscheduled work hours

Must have the ability to take direction, prioritize,



and work independently within multiple demands and within time restraints

Must maintain confidentiality of patient records, employee records and internal SNGR operations

Receives direction, instruction, guidance, support and supervision

Discusses individuals’ assessments, plans, progress and issues as they arise

Discusses and provides input to program planning and issue resolution

Provides input to program reports as required

Provides information/assistance as requested in a polite skilled manner

Works as a team player, supporting team members physically, emotionally, mentally and spiritually in our day-to-day operations, in a professional manner

KNOWLEDGE AND SKILLS

Demonstrated understanding of social and economic issues relating to people experiencing poverty including intergenerational trauma and mental health challenges

Commitment to SNGR values, culture, community bylaws and standards

Demonstrated experience providing cultural safe care within a First Nations community

IMPACT OF ERROR

Errors in carrying out duties can undermine the program and or Six Nations Council credibility, leading to poor public relations, lack of trust and potential embarrassment for the program, council and community. Such errors can result in client complaints that consume time and resources through administrative reviews and appeals, further eroding public confidence. Additionally, errors may cause harm to fellow employees and the public and could result in a loss of program funding due to non‑compliance with provincial and federal funding agreements.

CONTROL

Works within the administrative policies and procedures established by the Six Nations Elected Council and other legislation provided by various government agencies.
