This full-time position in the Division of Rehabilitation allows you to join a team of six dedicated physicians to care for 90 patients. Your daily responsibilities will include patient assessments and participation in on-call duties, emphasizing safety and quality in patient transitions. Familiarity with hospital patient management is advantageous.

Key Responsibilities:



• Provide comprehensive rehabilitation services to patients • Develop and implement individualized care plans • Monitor patient improvement in mobility and independence • Ensure safe transitions across the continuum of care • Collaborate on evidence-informed practices for quality outcomes

Requirements: • Certificate of Registration from the College of Physicians and Surgeons of Ontario • Certification in Family Medicine from the College of Physicians of Canada • Strong communication skills in English • Proficiency in additional languages is an asset • Engagement in academic activities required
