As part of Ontario Shores, you will provide comprehensive assessments and patient-centered treatment within the Geriatric Transitional Outreach Clinic.

Your work will help address complex mental health issues and improve care for patients aged 65 and older. Collaborating with a dedicated team, your expertise will guide treatment plans and foster effective family communication. Key Responsibilities:

- Lead psychiatric, medical, and ongoing cognitive assessments
- Safely prescribe and monitor therapeutic medications
- Develop and implement treatment and rehabilitation plans




- Educate families on diagnosis and care options
- Engage with community partners for integrated patient support Requirements:
- Current registration as RNEC with no practice restrictions
- At least 2 years of Geriatric/Mental Health experience
- Graduate degree preferred
- Knowledgeable in mental health and geriatric nursing
