Key Responsibilities:



• Deliver autonomous care across the full continuum of patient transitions • Collaborate with teams to enhance patient flow • Act as a primary point of contact for care coordination • Facilitate transfer of care when needed • Engage in leadership and education initiatives for nursing staff
