Squamish Nation is seeking a Home Care Nurse Assessor to join the Yúustway Health and Wellness Department within the Nexwnínlhewá7nem Ch'áwch’aw People Services Division.This role plays a vital part in supporting the health, independence, and wellbeing of community members by coordinating and assessing home care services. More than a clinical role, this is an chance to deliver culturally safe care and build meaningful relationships with clients, families, and community partners.What You’ll DoDevelop and implement client-centered care plans with input from families and community supports.Coordinate home care services in collaboration with the Home and Community Care team and allied health professionals.Conduct client assessments and maintain accurate, confidential health records.Provide clinical guidance and supervision to Health Care Assistants and Homemakers.Build and maintain strong relationships with internal teams, external providers, and community partners.Advocate for clients and support access to appropriate health services and referrals.Participate in program planning, development, and evaluation activities.Promote culturally safe, respectful,
