This part time independent contractor position offers 22.5 hours weekly, centered on assessing and coordinating care for residents during their move-ins and move-outs. You will also assist families in accessing necessary resources, ensuring residents have the support they need for a seamless transition.

Key Responsibilities:
Evaluate and assist residents in transitions
Document assessments and provide follow-up support
Collaborate with interdisciplinary teams
Facilitate Memorial Services with Recreation Department
Engage in quality improvement initiatives

Requirements:
Bachelor's degree in Social Work
Current registration with BC Association of Social Workers
