Become a vital part of the rehabilitation team at William Osler Health System as an Associate in Family Medicine. Contribute to patient recovery and independence through collaborative care strategies. This full-time position in the Division of Rehabilitation allows you to join a team of six dedicated physicians to care for 90 patients.

Your daily responsibilities will include patient assessments and participation in on-call duties, emphasizing safety and quality in patient transitions. Familiarity with hospital patient management is advantageous. Key Responsibilities:

- Provide comprehensive rehabilitation services to patients
- Develop and implement individualized care plans
- Monitor patient improvement in mobility and independence




- Ensure protected transitions across the continuum of care
- Collaborate on evidence-informed practices for quality outcomes Requirements:
- Certificate of Registration from the College of Physicians and Surgeons of Ontario
- Certification in Family Medicine from the College of Physicians of Canada
- Strong communication skills in English
- Proficiency in additional languages is an asset
