Job Summary

We are seeking a dedicated and compassionate Practitioner to join our dynamic healthcare team. In this vital role, you will provide high-quality patient care across various settings, including acute care, pediatrics, and home care environments. Your expertise will support patient recovery, pain management, and overall well-being through comprehensive clinical assessments, medication administration, and data collection. Duties
Conduct thorough patient assessments, including medical history and current health status, utilizing medical terminology accurately.
Administer medications safely and effectively according to established protocols and physician orders.
Collaborate with multidisciplinary teams to develop and implement individualized care plans.
Document all patient interactions, treatments, and outcomes meticulously within electronic health record systems.
Collect and analyze clinical data to monitor patient progress and inform treatment adjustments.
Provide specialized care in areas such as pain management, pediatrics,



or acute care settings.
Support patients with home care needs by educating them on medication use, self-care techniques, and follow-up procedures.

Requirements
Proven experience working in hospital or acute care settings with a robust understanding of medical terminology.
Prior experience in research or clinical studies is advantageous.
Demonstrated ability to deliver patient-centered care with empathy and professionalism.
Knowledge of electronic health records systems such as Epic or Cerner is highly preferred.
Experience in medication administration, pain management, and pediatric care is desirable.
Strong communication skills for effective collaboration with healthcare teams and patients.
Ability to collect accurate clinical data and maintain detailed documentation.
Previous work in home care environments is a plus.
