Key Responsibilities:
Provide comprehensive psychosocial assessments across different units
Collaborate on discharge planning initiatives with healthcare teams




Assist patients and families in accessing community resources

Supervise social work students during their practicum

Requirements:
MSW from an accredited Social Work program required
Current or eligible membership with OASW and OCCSWSSW
Experience in discharge planning and acute care settings preferred
Proficient in electronic health documentation
Solid interpersonal and decision-making skills
