Transform lives at the Jean Tweed Centre as an Advanced Practice Aftercare Counsellor. In this hybrid role, provide vital aftercare support to women exiting intensive treatment programs across Toronto. You will deliver evidence-informed aftercare groups while ensuring clients have access to necessary community services.
This full time, 18-month contract requires your experience in mental health and substance use, focusing on women’s individual needs. Engage in monitoring recovery progress and reducing client isolation through meaningful follow-up interactions. Key Responsibilities:
Launch and facilitate targeted aftercare groups for clients
Monitor client recovery progress through check-ins
Connect clients with local resources and services
Collaborate with teams to support integrated client care
Document interactions and prepare reports when needed Requirements:
Master’s degree in counseling or related field
Minimum 3 years in therapeutic settings
Proven experience in group facilitation
Robust understanding of community resources
