Para Med Niagara-on-the-Lake is seeking a Clinical Planner (RPN/LPN) to onboard recent patients, assess needs, implement and evaluate care plans, and coordinate district-wide services.
You will conduct initial safety and living-workplace assessments, schedule visits, maintain records, and inform patients about policies and timelines.
Requirements: Ontario RPN/LPN registration, at least two years' nursing experience, access to a vehicle, and the ability to pass a VPS background check.
