The Opportunity
Country Roads Community Health Centre (CRCHC) is seeking a Nurse Practitioner to join our interdisciplinary primary care team serving the rural communities of Rideau Lakes, Westport, Leeds and Thousand Islands, and surrounding areas. As the Most Responsible Provider (MRP) for an assigned roster of clients, you will provide comprehensive primary care across the lifespan, building meaningful long-term relationships while working to your full scope of practice.
Working alongside physicians, nurses, social workers, dietitians, pharmacist, and community health workers, you will deliver collaborative, client-centred care that addresses both medical needs and the social determinants of health. This is an opportunity to practice in a supportive team-based environment, make a tangible impact on rural health outcomes, and focus on patient care rather than clinic overhead or administrative burdens.
Key Responsibilities
Independently maintain and manage a caseload of varying ages.
Provision of primary care services including assessment, diagnoses, and treatment, chronic disease prevention and management, health promotion and counseling.
Collaboration and consultation with team members as necessary.
Maintain accurate and current client documentation using electronic medical records system.
Participate in case conferences, team,
and staff meetings.
Work within an inter-professional team to provide quality evidenced based client care.
Working in a manner that incorporates a health promotion approach supporting the social determinants of health.
Qualifications
Baccalaureate or Master of Nursing degree with successful completion of a post graduate Primary Care Nurse Practitioner program
Good standing with the College of Nurses
Current membership with NPAO, including professional liability insurance
Experience with Primary Care, preferably in a community setting
Demonstrated commitment to an inter-professional team environment
Experience with PS Suites, considered an asset
Compensation & Benefits
Compensation: $114,200 to $137,000
Benefits: Health & dental plan, HOOPP, health spending account, comprehensive leave benefits
Schedule: 35 hours/week, flexible schedule, on-call approx. 4 weeks per year
