Key Responsibilities:
• Support oncology patients navigating their care journey
• Facilitate connections with primary care providers
• Create and evaluate survivorship care programs
• Educate patients on available resources
• Collaborate with multidisciplinary teams on patient needs
Requirements:
• Active Registration with the College of Nurses of Ontario
• Five years of experience in oncology nursing
• Completion of Chemotherapy & Biotherapy Course
• Solid organizational and communication abilities
• Preferred qualifications include a Master’s degree
