Salary: $28.66 to $37.74 per hour, based on experience Join Our Primary Care Team

Sherwood Park-Strathcona County Primary Care Network is seeking an experienced and patient-focused Licensed Practical Nurse (LPN) to support the delivery of comprehensive primary healthcare services within a family medicine clinic.

In this role, you will work alongside family physicians and an interdisciplinary healthcare team to develop and implement care plans and provide follow-up with patients. The successful candidate will provide clinical care, health education, chronic disease support, preventative screening, and care coordination to help patients achieve their health goals. Duties will vary depending on the clinic and patient population.

Position Responsibilities
As a valued member of the clinical team, you will:

Deliver direct patient care within your LPN scope of practice

Administer immunizations and injections

Perform wound care and dressing changes

Conduct ear irrigations and support women's health services, including Pap testing

Provide diabetes education and prenatal support

Assist with patient assessments and follow-up care

Monitor patient progress and communicate findings to physicians and care team members

Support patients in understanding treatment plans and self-management strategies

Participate in preventative health initiatives and screening programs





Coordinate referrals and help patients access community and healthcare resources

Maintain accurate and timely documentation within electronic medical records

Support quality improvement activities within the clinic

Assist with orientation and training of new team members when required

Contribute to efficient patient flow and clinic operations

QualificationsRequired

Licensed Practical Nurse in good standing with the College of Licensed Practical Nurses of Alberta (CLPNA)

Current Basic Life Support (BLS) certification

Minimum three years of nursing experience

Strong understanding of evidence-based nursing practice

Experience working in a collaborative healthcare environment

Preferred

Primary care or family practice experience

Chronic disease management experience

Community health, gerontology, or preventative care experience

Familiarity with EMR systems

Experience providing patient education and health coaching

Current Criminal Record Check and Vulnerable Sector Check satisfactory to the organization

Valid Alberta driver's license

Access to reliable transportation as travel may occasionally be required
