Become a vital part of the rehabilitation team at William Osler Health System as an Associate in Family Medicine. Contribute to patient recovery and independence through collaborative care strategies.

This full time position in the Division of Rehabilitation allows you to join a team of six dedicated physicians to care for 90 patients. Your daily responsibilities will include patient assessments and participation in on-call duties, emphasizing safety and quality in patient transitions. Familiarity with hospital patient management is advantageous.

Key Responsibilities:
• Provide comprehensive rehabilitation services to patients
• Develop and implement individualized care plans
• Monitor patient improvement in mobility and independence




• Ensure safe transitions across the continuum of care
• Collaborate on evidence-informed practices for quality outcomes

Requirements:
• Certificate of Registration from the College of Physicians and Surgeons of Ontario
• Certification in Family Medicine from the College of Physicians of Canada
• Strong communication skills in English
• Proficiency in additional languages is an asset
• Engagement in academic activities required
