Your Opportunity:

Recovery Alberta's Addiction and Mental Health Supported Living program is seeking an experienced and highly skilled Social Worker II to join our Flow team. The Flow team focuses on improving the patient flow out of hospitals and into AMH supported community housing programs. This involves an in-depth assessment of patients’ needs and abilities, a thorough understanding of AMH presentations, and the ability to match the patient profiles with the appropriate program. The team acts as a liaison for inpatient teams back out to the community to ensure housing placements are done as seamlessly as possible. This is a specialized position that requires a high level of knowledge of all the resources across the broader housing sector. Essential to the skill set is a comprehensive awareness of personal and systemic housing barriers. The incumbent will have a thorough knowledge of concurrent practice and be able to demonstrate this knowledge through case presentations back to the team and housing operators. The Flow team also fields inquiries from the community as well as participates in complex case reviews from our internal partners as needed. This is a quick paced, high demand position and requires an ability to mediate high pressure situations as well as an ability to think creatively when looking at housing options. Robust boundaries and time management skills are critical to success. The position involves time spent in hospitals meeting with both clients and inpatient teams. Travel throughout the Edmonton area will be required.

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include:



psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications:

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Additional Required Qualifications:

Current CPR (BLS Level C – Health Care Provider). NCI - Non-Violent Crisis Intervention. A driver's license and access to a working personal vehicle and willingness to transport clients as required. 3-5 years' experience in a community mental health setting. Extensive knowledge of the housing sector. Cultural competence and a commitment to ongoing reconciliation and cultural safety. Flexibility, openness, and a desire to be a strong team player.

Preferred Qualifications:

Relevant experience in AMH Supported Living (or equivalent programs) and an understanding of the intake process for supported living sites will be a strong asset. Excellent communication and self-management skills. Knowledge of Concurrent Capable practice. Knowledge of Recovery Oriented Care. Experience working with multiple partners.
