PQWCHC is a multi-sited organization.General ResponsibilitiesWe are seeking an experienced Physiotherapist who will deliver programs and services to clients with musculoskeletal conditions as well as chronic illnesses, including COPD and physical conditions related to mental illness. Experience with pelvic rehab and programs for people who are obese would be an asset. The physiotherapist will work within an inter-professional team to provide comprehensive assessment and treatment according to the standards of practice of the College of Physiotherapists of Ontario.Reporting ManagerManager, Primary CareSpecific ResponsibilitiesDevelop care plans by assessing, developing, implementing and evaluating treatments for clients with acute and chronic musculoskeletal or neurological dysfunction, according to the College of Physiotherapists of Ontario guidelines and best practicesEnsure continuum of care is maintained by educating clients and family members, if appropriate,



with regard to the management of health care problems and re-evaluating physiotherapy programs considering the context of the clients’ home environmentProvide consultation and capacity building to other centre staff and community agencies working with clients who may need physiotherapy careDevelop, implement, monitor and evaluate, education and health promotion programs for individuals, families and the community, in order to foster self-help, mutual aid, and capacity building in the area of prevention or management of musculoskeletal and neurological problems according to community needsAssess for assistive devices (mobility equipment) and complete applications, where appropriate, for the Assistive Devices ProgramDevelop and maintain formal and informal networks within the community to optimize health care service provision and address needs of communityMaintain complete and accurate client records, assessment notes and required correspondenceParticipate in chart reviews and case conferences, and make appropriate internal and external referralsParticipate in the Physiotherapy Network, including monthly meetings,



resource development and quality improvement initiatives.QualificationsMaster of Science in Physiotherapy from a recognized university and/or an equivalent combination of relevant education and experienceRegistered under the College of Physiotherapists of OntarioEligibility for membership with the Canadian Physiotherapy AssociationBroad range of clinical experience (including chronic disease management) with different populations using a variety of modalitiesTraining in manual therapy, acupuncture and/or dry needling considered an assetDemonstrated ability to work effectively in an inter-professional team environmentExperience in the use of computers and various software applicationsExperience in program development, implementation, monitoring and evaluationADP authorization (or willingness to obtain) is considered an assetKnowledge and awareness of issues related to homelessness, immigration, mental health, poverty and discriminationExperience in a community-based setting working with marginalized groupsKnowledge of the health care/social service systems and resources available to clientsStrong communication, problem solving and conflict resolution skillsAbility to manage complex workload and remain flexibleFluency in languages other than English is an assetExperience providing care to Aboriginal clients is an assetExperience in Aboriginal approaches and healing practices is an assetCompensation$75,161 to $87,847 per annum plus Benefits and HOOPP (Healthcare of Ontario Pension Plan)Please also note that the successful candidate will be required to provide a police vulnerable sector check as a condition of employment, however solid consideration is made to the type of offense.
