Make a difference in Fort Saskatchewan as a Community Disability Support Practitioner. This full-time role involves empowering individuals with disabilities through skill-building and community initiatives.
Your mission will be to implement Individualized Service Plans and assist individuals with achieving personal and professional milestones. You will engage in assessing needs, facilitating job placements, and promoting secure interactions across various community settings. Collaboration with families and community partners will be essential to promote inclusion and self-advocacy.
Key Responsibilities:
• Develop and monitor service plans for individuals • Assist in job readiness and community access • Conduct evaluations of individual skills • Report safety hazards and ensure supportive environments • Communicate effectively with stakeholders
Requirements: • Rehabilitation Diploma or relevant degree preferred • Access to a reliable vehicle and valid driver's license • Excellent verbal and written communication skills • Capable of managing physical and emotional challenges • Positive attitude with coaching skills
