Elevate patient care as a Social Worker in the Nephrology Program at Humber River Health. This permanent, full-time role involves providing psychosocial support and coordinating care for patients living with chronic kidney disease. As a Social Worker, you'll play an essential role in managing patient care by conducting assessments, offering counseling, and collaborating with a diverse healthcare team.
You'll also engage community resources to support patient discharge and advocate for their needs throughout the treatment process. Key Responsibilities:
- Provide psychosocial assessments and counseling
- Plan and coordinate discharge for nephrology patients
- Serve as a resource to interprofessional team members
- Develop partnerships with community support services
- Manage patient caseload effectively in an acute setting Requirements:
- MSW from an accredited institution needed
- Membership with OASW and OCCSWSSW required
- Relevant nephrology and dialysis experience preferred
- Robust organizational and communication skills
