Salary Range

The salary range for this position is CAD $41.42 - $59.52 / hour

Detailed Overview

Reporting to the Manager, Clinical Services, the Indigenous Maternal Liaison provides support to life givers / patients / clients and their families during the perinatal care journey. The Liaison plans, organizes, and advocates for the health care needs and services of Indigenous patients and their unborn children throughout their pregnancy journey. Nursing care includes assessments, health teaching to patients / clients, support, consultation, liaising and referrals to appropriate services.

Works collaboratively with the inter-professional care team and the Surrey Memorial Hospital (SMH) Maternal Infant Child Youth (MICY) program in the provision of person-centered care. Facilitates connections between Indigenous patients and service providers in the community to enhance access to Fraser Health (FH) services. Provides support to SMH MICY staff on how to support life givers throughout the perinatal care journey with a culturally safe approach.

Responsibilities

- Acts as an Indigenous Maternal Liaison for life givers / patients / clients, families / support persons, service providers, and community agencies to enhance service delivery by working with life givers / patients / clients, families and service providers to develop a culturally protected care plan. Forwards necessary information to appropriate community services / agencies. Conducts patient assessments and screenings including evaluations of emotional, physical, spiritual and mental status of patient using information from all available sources.

Clarifies patient and family expectations, and acts as a patient advocate. Communicates with, and obtains assistance from, appropriate community agencies to ensure seamless care for patients and families after discharge from hospital.

- Collaborates and coordinates with life givers / patients / clients, their families / support persons and members of the perinatal and inter-professional care team in the development and ongoing modification of an optimal life giver and unborn baby care plan. Care team includes physicians, MICY manager, clinical leadership, MICY staff, Indigenous Primary Health and Wellness Home (IPHWH) staff, Fraser Region Indigenous Friendship Centre Associated (FRAFCA) staff, Birth Keepers, midwives, doulas, public health nurses, community health nurses and other health care professionals / providers.

- Establishes therapeutic relationships by demonstrating compassion, cultural safety and humility, trust and respect. Advocates for the life giver’s / patient’s / client’s rights to privacy, dignity and access to information.



Supports their choice and autonomy in decision-making and care planning. Supports life givers / patients / clients, their families / support persons at clinic visits as requested by life giver / patient / client.

- Teaches life givers / patients / clients and their families / support persons on pregnancy, the birth experience, postpartum, newborn care and parenting through a combination of clinic, telephone and/or home / outreach visits. Assesses learning needs of life givers / patients / clients and their families / support persons. Provides information on, and demonstrates when required, care activities to support self-care and the discharge plan.

- Provides maternal education and health promotion services to life givers / patients / clients, their families / support persons, and Indigenous communities to increase knowledge regarding maternal health through an Indigenous lens, promoting personal and family health and wellness.

- Participates in the care and discharge planning process. Collaborates with the inter-professional care team to ensure comprehensive care planning and delivery. Collaborates and communicates with the Patient Care Coordinator on issues affecting the care / discharge plan.

- Assists with arranging outreach nursing care to life givers / patients / clients in their home setting, primary care setting, or suitable alternative. Explains plan of care to life giver / patient / client and/or their family / support person. Identifies other care services required in accordance with applicable guidelines, policies, cultural protocols and evidence-based best practice.

Informs, educates, and provides health knowledge translation to life givers / patients / clients and their families / support persons, and checks for understanding as appropriate.

- In collaboration with members of the inter-professional care team, plans and discusses referral information with life givers / patients / clients and their families / support persons.

- Acts in public relations and advocacy capacity to represent IH to medical staff and other service providers, and to community agencies and their staff by maintaining up to date knowledge of IH. Collegially shares information to FH staff for the purpose of enhancing knowledge of Indigenous cultures, traditional practices and health beliefs, and providing culturally appropriate care.

- Provides presentations and educational programs for selected population groups via telephone, one-on-one and groups in clinical,



community and outreach settings. Provides information on services available in the community for Indigenous people.

- Participates in committees that integrate IH with hospital and community programs by providing input and awareness on Indigenous Health issues (e.g. participates in the Honouring Indigenous Perinatal Ceremony in Acute Care Settings Fraser Health Authority Steering Committee).

- Reviews and adapts relevant information materials related to providing culturally safe care. Participates in continuing education programs to keep up to date with current trends and research. Provides input on orientation of new staff, students, and other health care professionals to facilitate learning through an Indigenous cultural safety lens. Maintains knowledge of local community health services and resources.

- In accordance with established standards and procedures, maintains related records, documentation and statistics, and prepares related reports as required.

- Performs other related duties as assigned.

Qualifications

Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Two (2) years' recent, relevant nursing experience. Knowledge of Indigenous health approaches, protocol and cultural issues.

Valid BC Drivers Licence and have access to own vehicle.Skills and Abilities

- Demonstrated knowledge of Indigenous traditional, cultural and ceremonial practices related to birthing protocols and maternal care.

- Comprehensive knowledge of Indigenous health approaches, protocols, cultural practices, and community resources and agencies.

- Ability to deliver culturally sensitive services to Indigenous populations.

- Ability to promote person-centered care that demonstrates compassionate care for life givers/patients/clients and families/support persons, sensitivity to diverse cultures and preferences, and patient advocacy.

- Ability to assess, plan, implement, and evaluate.

- Ability to communicate effectively with life givers/patients/clients, families/support persons, the public, medical staff and members of the inter-professional care team using verbal, written and electronic communication means.

- Ability to teach and instruct life givers/patients/clients and families/support persons on topics essential to their health and wellness.

- Ability to work effectively with individuals, groups, and other professionals within a multi-disciplinary environment in a diverse community.

- Ability to prioritize workload, keep accurate records and write reports.

- Ability to operate related equipment, including personal computers and related software.

- Physical ability to perform the duties of the position.
