As a Float Hospital Care Coordinator, you will play a key role in developing comprehensive care plans tailored to individual patients, facilitating a smooth transition from hospital to home.

- Engage and assess clients for healthcare program admissions

- Implement and monitor client care initiatives
- Reassess services regularly to ensure quality care
- Manage discharge planning and resource redirecting Requirements:
- Degree in Nursing, Physiotherapy, or Social Work required
- At least two years of related healthcare experience
- Knowledge of local health care delivery systems
- Expected COVID-19 vaccination as per policy
