- Summary of Duties

As a member of the Assertive Community Treatment Team, the incumbent will provide client centred treatment, rehabilitation, support, and case management to clients in the community with severe & persistent mental illness.

Qualifications/Skills

- MSW, B.Sc. OT, M. Sc. OT, or COAT approved Equivalent required

- Member in valuable standing of the Ontario College of SW and SSW, or College of Occupational Therapist of Ontario required

- Recent community experience working with patients with severe and persistent Mental Illness

- Knowledge and comfort with functioning within the ACT model of Care

- Basic knowledge of psychotropic medications

- Ability to work independently and interdependently within a multidisciplinary team

- Excellent oral and written communication skills

- Demonstrated effective interpersonal skills

- Demonstrated problem-solving skills

- Commitment to client-centered care

- Knowledge of the health care system and community resources

- Valid Ontario Driver’s License required

- Ability to travel extensively within the catchment area
