Key Responsibilities:
• Create individualized support plans and goals for families
• Conduct annual reviews and compliance with regulations




• Provide resource information and links to families
• Monitor service quality through regular visits
• Facilitate financial discussions with families and partners
Requirements:
• Degree in social work or relevant diploma required
• Minimum 3 years' experience in developmental services
• Excellent interpersonal and organization skills
• Robust problem-solving and critical thinking abilities
• Valid driver’s license and access to a vehicle
