In this role, you will help manage care for approximately 200 residents, develop individualized care plans, and collaborate closely with families. Your background in gerontological and community nursing is key to ensuring every resident receives focused support.

Key Responsibilities:
• Deliver emergency and urgent care services
• Monitor clients with stabilized chronic illnesses
• Develop community educational programs




• Provide individual counselling for residents
• Work with a multidisciplinary team to support transitions

Requirements:
• Registration in valuable standing with the College of Nurses of Ontario
• Minimum 2 years in gerontological nursing
• 5 years in primary and acute care settings
• Robust interpersonal and problem-solving skills
• Proficiency in Meditech, Microsoft Word, and Excel
