and support to clients to assist in goal achievementWork in collaboration with partners to provide timely interventionsProvide discharge planningDocumentationComplete required documentation related to clients as per the organization’s standards, discipline-specific guidelines and standards and PHIPA legislationEnsure that confidential program/caseload information and material is handled in accordance with applicable privacy laws and the organization’s policyWhat you'll need to applyPolice Vulnerable Sector Check (PVSC)Successful candidates must have a PVSC completed within the last six months. Applicants who need a PVSC are encouraged to apply for one as soon as possible to avoid delays.QualificationsRPN, RN, or Bachelor's or Master's Degree in Occupational Therapy, Social Work, or Psychology or related field is required (primary proof of credentials will be required during the hiring process, e.G. original degree/diploma or official transcript)Currently registered and in good standing with the Ontario College of Social Workers and Social Service Workers (as an RSW), the College of Nurses of Ontario, the College of Registered Psychotherapists of Ontario, or the College of Occupational Therapists of OntarioValid driver's licence and access to a vehicle with at least $2 million in liability insurance to occasionally transport clientsCurrent, transparent Police Vulnerable Sector Check (PVSC)Ability to attend work regularlyPreferred qualificationsExperience supporting individuals with significant mental health concerns,
