Join Main Street Project as a Case Management Specialist focused on housing in Winnipeg. Engage with individuals to assist them in achieving safe and reliable housing through customized care.
In this full time role, you will be part of a dedicated team in case management focused on providing support through a housing-first and trauma-informed lens. Engage with participants, assess their needs, and develop tailored care plans. Your relationship-building skills will play a critical role in advocacy and resource connection for participants.
Key Responsibilities:
• Coordinate individualized care plans with participants • Build and maintain partnerships with community organizations • Conduct needs assessments and identify obstacles to housing • Facilitate regular check-ins to monitor progress • Provide crisis intervention support as necessary
Requirements: • Bachelor’s Degree or equivalent experience in social work • Proficiency in case management processes and documentation • Familiarity with harm reduction principles • Proven experience working with Indigenous groups • Excellent written and verbal communication skills
