Pay RangeEstimated annual earning potential: $80,000 - $130,000. Actual earnings may vary based on availability and service volumes.Job DescriptionAs a Dietitian at Champlain Rehab Solutions, you will be assessing and providing consultation, and recommendations of dietetic interventions to adults and/or geriatric group of clients in their homes using evidence-based best practice. You will ensure coordinated delivery of care through ongoing communication with the client care team regarding plans and schedules for interventions, problem solve in collaboration with the client and care team to create opportunities for more independence and promote CRS’s core beliefs and values.Key Areas of AccountabilityPrepares client reports for health professionals to analyze for treatment.Prepares plans and presentations for clients and their caregivers to provide thorough information on their diet as well as their goals.Educates internal and external staff members about healthy eating and proper nutrition.Supports the following areas of intervention: failure to thrive, dysphasia, feeding problems, enteral feeding, diabetes, cancer, GI disorders, weight management, and palliative care.Helps patients coming to terms with their illness and advise about special and time‑specific diets.Supports protected client care through the development of knowledge and skills in accordance with our principles of continuous quality improvement.Participate in a weekend on‑call roster within the geographical region to provide services based on client needs.Required QualificationsMember in good standing with the College of Dietitians of Ontario.Minimum 2 years clinical experience (optimal).Valid Driver’s License with access to a vehicle.Compassionate and empathetic.Self‑driven with ability to work independently.Skilled in caseload management.Skilled at electronic documentation.Able to provide virtual care.Compensation and BenefitsCompensation for education and professional development.CRS is a leading learning organization so we can provide you with comprehensive orientation and training at the start and throughout your career.Mentorship and peer support.Career development opportunities.Employee and family assistance program.Wellness resources.Perks & discounts.Staff & service provider events.Accommodation and Commitment to DEIAt Champlain Rehab Solutions (CRS), we passionately strive to uphold our commitments to being an Equal Opportunity Employer.
