What you will do
Undertake Care Act assessments, reviews, and support planning using a strengths-based, person-centred approach.
Facilitate safe and timely discharges from acute and community hospitals in line with the Discharge to Assess model.




Work collaboratively with NHS colleagues, therapists, community providers, carers, and voluntary sector organisations to achieve positive discharge outcomes.
Complete Mental Capacity Act assessments, make best interest decisions, and contribute to safeguarding enquiries as needed.
Assess risks, balance individual choice with duty of care, and develop appropriate risk management plans.
Qualifications
Degree in Social Work, CQSW, DipSW or an equivalent recognised qualification.
Evidence of continuing professional development.
Registration & Compliance
Current registration with Social Work England (SWE).
Experience
Post-qualification experience within Adult Social Care.
Experience working within hospital discharge, community social work, reablement, or integrated health and social care services.
