As a full-time Nurse Practitioner, you will serve as the Most Responsible Provider for your patients, working closely with a dedicated interprofessional team. The role includes using PowerChart for documentation and engaging in 12-hour shifts while receiving mentorship to ease your transition to an acute care environment.

Key Responsibilities:
• Deliver acute care within the home for assigned patients
• Serve as MRP, ensuring coordinated patient care
• Collaborate with an interprofessional healthcare team




• Manage clinical documentation using PowerChart EMR
• Participate in rotating shifts for continuous patient care

Requirements:
• Valid Nurse Practitioner license and qualifications
• Experience with acute care or in-home treatment
• Excellent cooperative and communication skills
• Familiarity with electronic medical record systems
