This full time position requires you to work closely with participants to develop, implement, and evaluate their individualized housing goals. Rooted in a harm reduction approach, you will collaborate with various community resources, assist in crisis situations, and prioritize relationship building to ensure participant success.
Key Responsibilities:
• Implement individualized care plans with ongoing support
• Identify and address barriers to housing for participants
• Create a welcoming environment for tenant concerns
• Keep accurate records and documentation of progress
• Attend community and coalition meetings related to housing
Requirements:
• Bachelor’s Degree with relevant frontline experience
• Strong understanding of case management and care planning
• Experience with culturally diverse populations
• Crisis intervention and management skills
• Ability to work independently and in teams
