Position Summary
As a member of an interprofessional team, the Registered Practical Nurse (RPN) with the Alternate Health Facility (AHF) Program provides patient-centered nursing care, supporting care needs of patients in the retirement home within the Orillia Soldiers’ Memorial Hospital (OSMH) supported beds. The RPN supports the assessment, planning, implementation, and evaluation of care for assigned patients. Working under the direction of medical practitioners, Registered Nurses (RNs), and other regulated health care professionals, the RPN practices within scope in accordance with the College of Nurses of Ontario standards, Hospital policies, and Patient Care Services guidelines. The RPN delivers safe, compassionate, and evidence-informed care while contributing to patient safety, continuity of care, and collaborative practice. The RPN will network to establish relationships with partner organizations, the Couchiching Health at Home Team (COHT), and OSMH team members to enhance the care for complex elderly residents and their caregivers to further support the AHF program.
Key Responsibilities
- Meets the College of Nurses of Ontario Standards of Nursing Practice for Registered Practical Nurses and practices in accordance with Hospital and Patient Care Services policies and procedures.
- Provides nursing care with the Alternate Health Facility program, supporting patients within the retirement home within OSMH supported beds.
- Organizes and implements nursing care plans in accordance with established care plans.
- Performs nursing interventions including head-to-toe assessments, vital signs, applying aseptic techniques and sterile dressing changes, ensuring infection control, wound, phlebotomy, ostomy, catheter, and drain care, specimen collection, and medication review.
- Administers medication and monitors therapeutic effects and documents patient responses accurately and in a timely manner.
- Utilizes critical thinking to assess changes in patient status (complexity, stability) and initiates consultation, escalation, or transfer of care as appropriate. Receives written and verbal handover of patients admitted to the Alternate Health Facility (AHF) program as well as provides handover to facilities at the time of discharge.
- Supports safe transitions of care through patient and family education, reinforcement of discharge plans and referrals to appropriate community resources.
- Provides cognitive, social, emotional, and spiritual support to patients and families.
- Utilizes human and material resources effectively and responsibly.
- Participates in quality improvement activities, audits, and program initiatives to enhance patient safety and care outcomes.
- Participates in the orientation and mentorship of current team members and nursing students, including preceptorship activities as assigned.
- Maintains professional competence through ongoing learning, attendance at education sessions and in-services, and self-assessment of professional development needs.
- Responsible for contributing to patient safety and promoting a culture of safety.
- Functions in accordance with established hospital emergency procedures.
- Participates as a member of the interdisciplinary team, promoting effective liaisons between patients, family and physicians, healthcare team members, and community agencies. Encouraging referrals to appropriate community resources.
- Maintains awareness of current developments in nursing practice and relevant legislation, including the Regulated Health Professions Act (RHPA).
- May be utilized in other patient care areas during a shift based on patient care needs and operational requirements.
- Performs other related duties as may be assigned by the Program Manager or designate.
Qualifications
Education
- Registered and in good standing with the College of Nurses of Ontario (CNO) as a Registered Practical Nurse is required.
- Current Basic Cardiac Life Support (BCLS) is required. CPR Level HCP (Health Care Provider) will be accepted as an equivalent.
- IV certification or the ability to obtain certification within six (6) months of hire is required. Experience with IV initiation may be considered.
- Successful completion of Phlebotomy certification is required.
- Successful completion of aseptic principles certification is required.
- Gerontology Nursing Certification is preferred.
Experience
- A minimum of two (2) years of recent experience in an acute care or long- term care setting is required.
- A minimum of two (2) years of recent home care experience is preferred.
- A minimum of one (1) year experience with wound care, ostomy care, and catheter care is required.
