Help people in Kirkland Lake and North Timiskaming stay in their own homes.
Timiskaming Home Support is hiring a Registered Practical Nurse to assess clients at home, coordinate the services that keep them there, and guide the Personal Support Workers and Home Help Workers who deliver that care. It is nursing judgment, team leadership and community connection in one job.
What you will do
- Assess clients in their homes and arrange services around each person’s needs, preferences and goals; review plans regularly and adjust as needs change.
- Supervise day-to-day service delivery and quality for the PSWs and Home Help Workers in your region, together with the Team Leads, under our dual-oversight model; coach staff, teach special functions and help orient new hires.
- Resolve service complaints, refer clients to partner agencies and follow up to make sure the placement worked.
- Keep client files and activity data current and confidential in AlayaCare, and provide the data behind our funder reporting.
- Help plan services and programs; work with the other Coordinators, Team Leads and Service Planners on scheduling and coverage; take your turn on the on-call rotation.
- Contribute to quality improvement, including our first CARF accreditation.
About 75% of your time is in clients’ homes, 20% in the Kirkland Lake office and 5% on the road.
What you bring
- RPN with current registration in good standing with the College of Nurses of Ontario
- Strong knowledge of community health services and programs
- Program evaluation and outcome-measurement experience
- Excellent interpersonal, communication and organizational skills; effective communication in English
- Sound judgment and the ability to work with minimal supervision
- Strong computer skills, including client-management software
- Valid driver’s licence and access to a reliable vehicle (mileage reimbursed)
- Satisfactory Criminal Record Check with Vulnerable Sector Screening
Assets: French (THS is a designated French Language Services agency), supervisory or leadership experience, and community involvement. If you hold your CNO registration and meet most of the rest, please apply.
