Key Responsibilities:



• Provide comprehensive psychosocial assessments across different units • Collaborate on discharge planning initiatives with healthcare teams • Assist patients and families in accessing community resources • Flexible management of caseloads in high-pressure situations • Supervise social work students during their practicum

Requirements: • MSW from an accredited Social Work program required • Current or eligible membership with OASW and OCCSWSSW • Experience in discharge planning and acute care settings preferred • Proficient in electronic health documentation • Strong interpersonal and decision-making skills
