Nurse Practitioner – Full Time

Full time: Permanent (35 hours per week)

Schedule: Monday to Friday

Start date: October 2026

Salary range: ($67.13 - $68.94/ hour)

Reports to: Nurse Practitioner - Lead The GNPLC is a dynamic community-based, multi-disciplinary primary care clinic providing comprehensive team-based health services to residents of SDG with particular expertise in providing care and access to people in the communities focusing on high quality care and social determinants of health.

The Glengarry Nurse Practitioner-Led Clinic (GNPLC) is dedicated to providing accessible, person-centered, and comprehensive medical care. Their mission is to offer high-quality, client-centered health services using a team-based approach, ensuring that clients receive the highest standard of care. The vision of the GNPLC is to respond with empathy and understanding, to meet each person's needs across their lifespan, continually seeking new solutions and creative methods to enhance care and services.

They commit to building a strong partnership with clients, families, and community partners to provide comprehensive individualized care.

Nurse Practitioner Responsibilities:

· Performing comprehensive health or symptom directed assessments of clients to make a diagnosis.

· Ordering and interpreting the results of diagnostic laboratory and screening tests within scope of practice.

· Initiating, managing and monitoring the ongoing therapy(s) of clients with chronic illness by providing effective pharmacological, complementary, or counseling interventions, within a client-centered and evidence-based framework.

· Diagnosing and treating acute/episodic minor illnesses and injuries.

· Collaborating with medical specialists, sector partners and inter-professional team members to develop individualized care plans based on evidence based clinical best practices.

· Maintaining a preventive health monitoring program for clients (e.g cancer screening, BP etc.)





· Providing care in accordance with the Standards of Practice for Registered Nurses in the Extended Class and related Acts.

· Ensuring occupational health and safety including infection prevention and control measures are practiced.

Qualifications:

· Current certificate as a Nurse Practitioner-RN (EC) and registered in good standing with the Ontario College of Nurses (CNO).

· Maintains professional liability protection (PLP) with CNPS.

· Possesses relevant clinical experience (assessment, diagnosis and treatment services) within a Primary Health Care setting.

· Demonstrates exceptional understanding of current, evidence-based methods and practices of primary health care delivery.

· Has experience managing chronic diseases (e.g., diabetes, hypertension, COPD), mental health concerns, and providing preventative care.

· Has additional training or credentialing (e.g. Addictions Treatment, Mental Health Experience, Certified Diabetes Educator, Certified Asthma educator, etc.) which are considered an asset.

· Is proficient with Electronic Medical Records (EMR’s) and community referral networks.

· Works collaboratively in an interdisciplinary team setting.

· Possesses superior interpersonal, time management and attention to detail skills and

· Speaks English proficiently with an ability to communicate in French.
