Overview

MedEx Health Services Inc. is seeking an experienced Registered Nurse (RN) or Registered Practical Nurse (RPN) to support the clinical delivery, staffing and coordination of our growing home and community healthcare services.

IMPORTANT – EXPERIENCE REQUIRED

Applicants must have a minimum of two (2) years of direct experience working in home and/or community healthcare, including care coordination.

This is an experience-required position. Candidates without the required community/home-care and coordination experience should not apply.

We are looking for an RN or RPN who combines strong clinical knowledge with demonstrated experience in home-care operations, community healthcare, staffing coordination, scheduling, patient navigation and interdisciplinary care.

Duties

- Key Responsibilities

The successful candidate will:
- Coordinate home and community care services, including nursing, PSW and rehabilitation services.
- Support staffing, scheduling, caseload management and service coverage for community-based clients.
- Participate in client intake, assessment, care planning, reassessment and ongoing service coordination.
- Coordinate and communicate with nurses, PSWs, rehabilitation professionals and other members of the interdisciplinary care team.
- Monitor client needs and ensure appropriate services and staffing are in place.
- Communicate with physicians, hospitals, community agencies, retirement and long-term care facilities and other healthcare partners.
- Support hospital-to-home and community transitions and help prevent gaps in care.




- Identify changes in client condition and ensure appropriate clinical escalation and follow-up.
- Support quality assurance, clinical documentation, policies and regulatory compliance.
- Develop and maintain strong relationships with community healthcare partners.
- Participate in the implementation and coordination of new home, community and transitional-care programs

Experience

- Mandatory Qualifications
- Current registration and valuable standing as a Registered Nurse (RN) or Registered Practical Nurse (RPN) with the College of Nurses of Ontario (CNO).

Previous experience with healthcare staffing, scheduling, workforce coordination and home-care operations is strongly preferred.

- Minimum two (2) years of direct experience in home and/or community healthcare.
- Minimum two (2) years of demonstrated experience in care coordination, community coordination or similar healthcare coordination responsibilities.
- Demonstrated experience coordinating services for clients receiving care in their homes or communities.
- Experience working with interdisciplinary healthcare teams.
- Strong knowledge of community resources and Ontario's healthcare system.
- Strong organizational, communication, leadership and problem-solving skills.
- Ability to manage multiple clients, staff schedules and operational priorities simultaneously.
- Strong computer and clinical documentation skills.
- Valid Ontario driver's licence and ability to travel within the community as required.

Pay: $35.00-$45.00 per hour
