Job Summary

We are seeking a dedicated and compassionate Registered Practical Nurse (RPN) to join our healthcare team. The ideal candidate will possess a strong background in nursing with experience across various care settings, including long-term care, acute care, and community-based services. As an RPN, you will deliver comprehensive patient and resident care, ensuring safety, comfort, and well-being while adhering to professional standards and organizational policies.

Duties

- Provide direct nursing care to residents and patients in accordance with individualized care plans and medical directives.
- Administer medications, including injections and IV therapies, following established protocols to ensure safety and accuracy.
- Monitor patient conditions, document findings accurately using electronic health records such as Epic or Cerner systems, and report changes promptly to the healthcare team.
- Assist with activities of daily living, including mobility support, hygiene, nutrition, and comfort measures.
- Collaborate with interdisciplinary teams to develop, implement,



and evaluate personalized care plans tailored to each individual’s needs.
- Support dementia care initiatives by implementing specialized strategies that promote safety and quality of life for residents with cognitive impairments.
- Participate in quality improvement activities, infection control protocols, and emergency response procedures to maintain a safe environment.

Skills

- Proven experience working in nursing homes, hospitals, or acute care environments with a focus on patient-centered care.
- Strong knowledge of medical terminology, medication administration procedures, and clinical best practices.
- Experience with electronic health record systems such as Epic or Cerner is highly desirable.
- Skilled in phlebotomy and laboratory procedures relevant to nursing practice.
- Excellent communication skills for effective interaction with patients, families, and healthcare team members.
- Ability to manage complex cases involving pain management, hospice care, or senior living environments.
- Familiarity with dementia care techniques, assisted living settings, pediatric patient needs, and home care services is advantageous.
