Job Summary
We are seeking a dedicated and compassionate Tawâw Case Manager to join our dynamic team. In this vital role, the Case Manager supports individuals accessing services through the Tawâw Permanent Supportive Housing program by providing culturally informed, client-centered case management. The role focuses on building trust, assessing needs, coordinating supports, and helping clients achieve stability in areas such as housing, health, income, and wellness.
Duties
- Conduct intake assessments and develop individualized service/care plans
- Provide ongoing case management and regular follow-up with residents
- Support residents in accessing income supports, healthcare, mental health, and addiction services
- Advocate on behalf of residents with community agencies, and resources
- Maintain accurate and confidential case notes, reports, and documentation
- Identify barriers to stability and collaborate on practical solutions
- Crisis intervention and de-escalation when required
- Support culturally safe care, incorporating Indigenous values, traditions, and community connections
- Participate in case conferences and multidisciplinary team meetings
- Build and maintain relationships with community partners and referral networks
- Transport residents to appointments and services when required
- Assist clients with mobility needs when entering/exiting vehicles if required
- Schedule and coordinate rides in alignment with client care plans
Skills:
- Strong case management experience with the ability to develop tailored support plans
- Proficiency in Microsoft Office Suite including Word, Excel, Outlook for data entry, documentation, and scheduling
- Exceptional organizational skills with effective time management to handle multiple cases efficiently
- Excellent interviewing skills to build rapport and gather detailed client information
- Ability to operate computers effectively for data entry, record keeping, and report generation
- Knowledge of community resources, social services systems, and advocacy techniques
- Effective organizational skills combined with the ability to prioritize tasks in a fast-paced environment
