Services may also include the provision of support groups. With considerable latitude and encouragement to initiate and work independently, the incumbent will be an adaptable and flexible person with the ability to work collaboratively in a variety of work settings. Social Workers are expected to exhibit sound professional judgement and initiative while working within the framework and objectives of Nova Scotia Health. Some home visits and travel are required.About YouWe would love to hear from you if you have the following:Master’s Degree in Social WorkRegistered or eligible for registration with the Nova Scotia College of Social Workers (NSCSW)Minimum of two (2) years clinical experience requiredEvidence of extensive knowledge of community resourcesEvidence of ability to work independently with a multidisciplinary team as well as with both hospital and community-based services/agenciesEvidence of ability to coordinate/develop/document and implement care plan for hospital discharge and safe transition back to the community or institutional settingDemonstrated use of innovative problem solving and application of evidence-based practice to patient servicesCompetencies in other languages an asset,
