Join Main Street Project as a Case Management Specialist focused on housing in Winnipeg. Engage with individuals to assist them in achieving safe and stable housing through customized care.
In this full-time role, you will be part of a dedicated team in case management focused on providing support through a housing-first and trauma-informed lens. Engage with participants, assess their needs, and develop tailored care plans. Your relationship-building skills will play a critical role in advocacy and resource connection for participants.
Key Responsibilities:
• Coordinate individualized care plans with participants
• Build and maintain partnerships with community organizations
• Conduct needs assessments and identify obstacles to housing
• Facilitate regular check-ins to monitor progress
• Provide crisis intervention support as necessary
Requirements:
• Bachelor’s Degree or equivalent experience in social work
• Proficiency in case management processes and documentation
• Familiarity with harm reduction principles
• Proven experience working with Indigenous groups
• Excellent written and verbal communication skills
