Vancouver Coastal Health is seeking a dedicated Mental Health Outreach Nurse. In this role, you’ll work to provide inclusive outreach services for those facing mental health and substance use issues.
This position requires a Registered Nurse or Registered Psychiatric Nurse committed to delivering integrated care across various settings. You'll assess client needs, intervene in crises, and coordinate care alongside interdisciplinary teams. Your work will directly impact the lives of those with significant health challenges in our communities.
Key Responsibilities:
• Assess client psychological and physical health
• Develop and implement client care plans
• Conduct counseling sessions to reduce healthcare reliance
• Participate in interdisciplinary team case meetings
• Provide outreach to connect clients with services
Requirements:
• BCCNM registration as RN or RPN
• Two years of related experience in mental health
• BC Driver's License for business use
• Knowledge of recovery models and treatment protocols
• Excellent written and verbal communication abilities
